Utility Bill Payment Plan Application

    I agree that my past due balance is the amount listed below. In order to pay this past due balance no later than February 1st, 2021 and bring my account current, I agree to minimum monthly payment shown below. I agree to pay my current utility bill plus the minimum monthly payment beginning on the date below. I understand that after August 1, 2020 (or termination of the Governor’s Executive Orders 124 & 142), my utility services may be disconnected until payment is made if I do not make the monthly payments agreed to in this payment plan.
